Instructions for thesis committee chairs: The purpose of this form is for thesis committees to provide students with feedback to help promote student growth and development and identify areas needing additional support or remediation. To submit the form, the chair of the thesis committee should complete each section of the form, enter the email address* of the student and each committee member, and click the submit button. After the form is submitted, it will be sent by email to the student, all members of the thesis committee, the EEB Graduate Program Administrator, and the EEB Director of Graduate Studies (*email addresses of the student and committee members are required in order for them to receive the form).
